Teachers

Step 1: Share this page with the parents of your students who will be attending the Georgia FFA-FCCLA Center. Preview the Teacher and Parent Resources to aid in the completion of the Online Medical Forms.

Step 2: Ensure parents have all required information to fill out the online medical form document. Utilize the Parent Instructions for the Completion of Online Medical Forms (See Below in Teacher Resources) as a resource.

Step 3: On the Online Medical Form, under School Information, a dropdown menu is present. The dropdown menu is labeled “School Name and Teacher Name.” Parents will be asked to select your school and teacher name from the dropdown menu. Upon selection of the school and teacher name from the dropdown menu, you will receive a confirmation email with a PDF copy of the student’s completed medical form.

Step 4: Track which students have completed the Online Medical Form by monitoring your emails.

Step 5: By Wednesday at 2:00 PM on the week prior to your week of camp, ensure that ALL of your registered students have completed the Parental Consent / Medical Information / and Student Code of Conduct.

NOTE: TEACHERS are responsible for ensuring that a medical form has been completed for each registered student attending Summer Leadership Camp.

Step 6: Review the forms and identify the students that will have medications that will need to be turned in, and also which students have allergies that you need to be aware of. Reviewing forms beforehand will help to ensure a speedy check-in on your week of SLC.

Step 7: Arrive at camp with medications to turn in! You do NOT have to bring a copy of the forms to turn in!
